Step 9 of the directions say "Put both the sealed metal bag and the completed Laboratory Request form into the pre-addressed Balance Test return envelope and seal." This step is causing some confusion, since there does not appear to be a Laboratory Request form. The only form that comes in the test is the Release form on the sample card, which goes in the metal bag. So you might want to correct step 9.
The Laboratory Request form is no longer in the box. You actually register that information at www.zinzinotest.com before the BalanceTest is mailed out. You can send in the BalanceTest without that form.

Yes, you would take the BalanceTest before you start taking the BalanceOil's. To see where you are at in Omega 6:3 balance before taking the product. As long as you remain on a monthly subscription, the free 2nd test will be shipped with the 4th monthly subscription order.
